The DUTCH-ICD study aims to refine risk stratification and improve clinical decision-making regarding implantable cardioverter-defibrillator (ICD) therapy in patients with non-ischemic cardiomyopathy (NICM), in order to select patients at highest risk.

Patients with NICM, LVEF <35% and myocardial fibrosis on Cardiac Magnetic Resonance Imaging (CMR) will be included and randomized 1:1. Patients in the intervention group will receive ICD implantation in addition to optimal drug therapy (OMT). Patients in the control group will receive OMT only. The DUTCH-ICD study thus investigates whether ICD implantation leads to a reduction in mortality compared to standard therapy without ICD in patients with non-ischemic cardiomyopathy, with demonstrated fibrosis on CMR and LVEF≤35%.
