It is known that many hospital admissions of older people are the result of prescribing errors due to long medication lists that
are composed by multiple doctors. In the LIMONCELLO (Less Is More: Optimized pharmacotherapy with improved coNtinuity of
CarE in hospitaLized oLder peOple) project, 7 Dutch University Hospitals work together with 9 Non-academic Hospitals to
find out how to prevent these hospital readmissions in patients who are currently admitted in hospital and have a high risk of
readmission. In this project, a dedicated team consisting of a doctor and pharmacist will optimise the medication list. This will be
done with close consultation with the patient and also the primary caregivers (general practitioner and community pharmacist).
The aim is that these patients are admitted less often, quality of life improves and it will also be checked if the number of
medicines could be reduced. This intervention will be compared to usual care.
